If you don't know your NHS number then ask our receptionist for it.  Do you know your NHS number?

Could we contact you in an emergency?  Do we have your current telephone numbers?

We need to keep up-to-date records for you, it is important that we have your correct contact telephone number and address.  If you need to update your details with us please ask for a change of details form from the Receptionist  the next time you visit us, or telephone the Receptionist on 01472 353303.

Would you like to become a member of our patient participation group?

Our patient group carries out some fantastic work on your behalf, but we need more members.  They would welcome any patient who can spare just an hour and a half four times a year.  If you would like to know more or would like to join, email Yvonne Waddingham at enquiries.pelham@nhs.net . 

The next meeting of the patient participation group is on Monday, 30th April, 2012 at 2.00 p.m.  Why not come along?
