For a number of years now the practice has been providing support to patients who were considered to be at risk of hospital admission. This service was possible through NHS enhanced service funding specifically for “Avoiding Unplanned Admissions”. With effect from 31st March 2017 the “Avoiding Unplanned Admissions” enhanced service will cease. This means that the practice will no longer be maintaining a specific register for patients considered “At Risk of Admission”. Patients on the previous register will no longer receive a follow up phone call of support when they have been discharged from hospital, however patients are encouraged to contact the surgery if they do require support. With effect from 1st April 2017 there is a new contractual requirement on “Identification and Management of Patients with Frailty”.
