Join the Practice

Anyone in England can register with a GP surgery. You do not need proof of address or immigration status, ID or an NHS number.

How to register

Our practice list is currently open and we are very pleased to welcome new patients. We are proud to operate a completely non-discriminatory acceptance policy.

If you would like to join our practice please complete the online registration following the link below or call in and visit our reception desk and ask for a new patient registration pack. An appointment for you to attend for a “new patient health check” will be made when the forms are returned completed. Alternatively please telephone our Reception Team on 01472 353303 for further information.

All we ask is that:

  • You reside within our catchment area (see below for boundaries)
  • That you have not previously been removed from our list due to unacceptable behaviour or poor attendance
  • You attend for a New Patient Health Check

Register online with Pelham Medical

We are using a new online service called Register with a GP surgery that makes it easy to register with this GP surgery.

Just fill in this quick online form to start the process. You do not need proof of address or immigration status, ID or an NHS number.

The service is designed and run by the NHS, so your personal information is safe. It cuts our administrative workload and makes it easier for you to register.

Paper forms are still available if you need one.

Church View area

Church View does not accept patients from:

  • Cleethorpes
  • Humberston
  • Waltham
  • New Waltham
  • Laceby & Laceby Acres
  • Aylesby
  • Healing
  • Stallingborough
  • Holton le Clay

All GP Practices are required to allocate a ‘Named Accountable GP’ to all patients. The allocated GP, will have overall responsibility for the care and support that our surgery provides to the patient. However, this does not prevent you from seeing any GP in the practice as you currently do. You are welcome to carry on seeing whichever GP you wish when booking appointments.
